Im pretty sure Hasek was Ed Belfour's backup in the 1991-1992 season, which saw the Hawks advance to, and lose in the Stanley Cup Finals. Along with the Sabres in 1999 and the Wings in 2002, that would make 3 finals appearances, and teams.

Dont think it's Hasek.

2. He played one season in Japan with Iwakura Tomakomai
3. Played under Joe Crozier in 79-80 with the New Brunswick Hawks and he was elected to the Manitoba Hall of Fame
4. Coached both the Hawks IHL and NHL teams?
5. Reached SCF as head coach with Kings and GM/Head Coach with Flames. However, he also reached it as an assistant with Blackhawks in 91-92

I accidentally stumbled on it.. Mike Babcock

1. Gold medal in the Winter Olympics (2010). Gold medal in World Championships (2004). Gold medal in WJC (1997). Stanley Cup Winner (2008). CIS University Cup winner (1994).

2. Played for the Whitley Warriors of the BHL before becoming a coach.

3. Played under Dave King at the University of Saskatchewan. Dave King is in the IIHF Hall of Fame as well as the Canadian Olympic Hall of Fame.

4. Coached Cincinnati Mighty Ducks as well as the Anaheim Mighty Ducks

5. Went to the SCF with both Anaheim and Detroit

Finally, I got one.
