I took an organizational psychology course back in college (early 90s) and we spent almost half the semester on the challenger disaster. NASA knew the o rings were a problem from previous launches and they pressured the o ring manufacturer to give them a thumbs up for launch. In a meeting at the manufacturer, the executives asked the engineers to take their engineering hats off and put their management hats on, or something like that. And they gave the ok to launch. By far the most interesting class I ever took.

What's truly sad is that they postponed the launch the first time because they thought it might rain ... it didn't. They wanted a nice sunny day for the world to watch. They came back the next day and couldn't get the shuttle door to close. They had to take some bolts off to fix it, and couldn't use any power tools (in case a spark caused an explosion from the tanks) - BUT all the battery operated tools in the mechanics toolboxes were dead and needed to be charged ! They blew through the 2nd attempt launch window and postponed it again, as the winds were too severe. They were fine before the stupid tool episode ! That night, Florida had temps in the 30's and the next morning, the shuttle was covered in ice ! They held the meeting you talked about and a couple higher-ups OK'd the takeoff, despite the shuttle being covered in icycles. The main NASA guy said "What, are you gonna wait 'til April to launch !?!" ... he was tired of waiting and signed off on the launch. We all know what happened next. The o-rings were brittle on takeoff and the rocket fuel bled through the cracks and ignited.
Truly sad and it didn't need to happen. They should have launched on the first attempt - who cares if it was cloudy. If the NASA maintenance idiots charged their cordless tools before the second launch, the door handle repair would have taken 15 minutes - not the hours they spent fiddling around with hacksaws trying to saw the door handle off - and they could have taken off during the launch window. The NASA incompetence was staggering. 2 avoidable launch postponements, then taking off on the 3rd attempt even though ice was all over the shuttle. |
