I’ve seen it one before in a game within the last 5 years or so. Player that was supposed to go to the box had to go to the trainers room to get stitched. Coach — I believe it was hawks and Q — elected to leave the box empty. When the penalty expired, since no one was in the box to come on the ice, the team had to wait for the first stoppage in play to put the 6th man back on the ice.
