That's because it's NFL policy. They have ridiculously strict clothing codes. I think it's stupid, but I believe it's part of their humongous big endorsement deals.

Players have requested wearing certain pieces of clothing for causes before and are routinely denied. Some guys do and just pay the fine.

I believe the operations manual states plays "should" stand, and players "may" receive discipline. However, players must be present on the sideline.

Correct. Other leagues do mandate standing for the anthem. The NBA has this mandate.

I agree with you that some of the blame falls on the players, but when its been 4 years its time to turn to the coaching staff IMO. They're the ones choosing the personnel to use on the PK. 4 years is more than enough time to acquire players, if needed, to at least be league average.
