It's a trust thing. This coach doesn't trust these players with the lead - they don't get it all the time, but when they do he has to let them play the way that got them the lead rather than packing it into prevent mode.

Its both. The coach is at fault for certain things but bottom line is the D isnt good enough to make up for the injuries on offence. They're compensating for it by having all 5 guys on the ice play a super defensive role, the moment they open it up theyd get exposed.
